Luther fogadója megtelik a Don Giovanni szünetében. Hoffmann, a költő kijelenti, hogy Stella, az ünnepelt sztár három korábbi szerelmét testesíti meg: a fiatal lányt, a művészt és a kurtizánt. El is meséli a három fantasztikus szerelmi kalandot:
Spalanzani feltalált egy mechanikus bábut, amit lányaként, Olympiaként akar bemutatni. Coppélius olyan szemüveget ad Hoffmann-nak, mellyel a babát embernek látja. Olympia bemutatása nagy sikert arat. Hoffmann szerelmet vall a bábunak, mire a vendégek kigúnyolják.
Crespel tanácsos felesége operaénekesnő volt és tüdővészben szenvedett. Lánya, Antonia örökölte édesanyja tehetségét, de sajnos betegségét is. Ezért Crespel megtiltja neki, hogy túlterhelje magát. Ekkor lép be dr. Miracle. Az orvos azt kezdi ecsetelni mennyivel ragyogóbb a színpad az egyhangú házaséletnél, és éneklésre szólítja Antoniát. A lány nem tud neki ellenállni, énekel, majd holtan esik össze.
Hoffmann és társa, Miklós a velencei kurtizán, Giulietta mulatságán vesz részt. Dapertutto megzsarolja Giuliettát, hogy csábítsa el Hoffmannt és rabolja el tükörképét, ahogy azt már korábban Schlemillel is megtette…
A záró képben Hoffmann újra a fogadóban ül részegen és az öngyilkosságon mereng. Megsérti Stellát, aki örökre búcsút int neki.
